Redefining Support for LGBTQ+ New Yorkers

At The Center, our case management team plays a crucial role in supporting LGBTQ+ New Yorkers as they navigate life's trials and tribulations. The team will conduct an initial assessment to understand your unique circumstances, challenges, and goals.

They will work with you to identify your specific needs and develop a personalized action plan. There are four case managers with specific areas of expertise: Immigration, TGNC Gender Affirming Care, Public Benefits & Housing Support. Our team works with community members to create an action plan.

Immigration

Center Support offers referral services to community members in need of immigration support.

  • Support in navigating case status with EOIR & USCIS
  • Access to immigration self-help resources
  • Assistance with enrolling in health insurance to ensure you have access to necessary medical care

TGNC Support

Our dedicated case coordinator provides gender-affirming services to support you in the following ways, regardless of immigration status.

  • Connections to TGNC-affirming medical care resources, including Hormone Replacement Therapy
  • Gender-affirming services to support you through name change referrals, health procedures, housing, food security, and cash assistance
  • Assistance with enrolling in health insurance to ensure you have access to necessary medical care
  • HIV and sexual health services

Public Benefits and Housing

Get Started

  • Support with Access HRA and Housing Connect
  • Assistance in navigating SNAP, Cash Assistance & other benefits
  • Support in accessing or maintaining resources that are LGBTQIA+ affirming, including assistance in securing affordable housing and shelter
  • Free events where you can access basic resources, such as food and clothing, to help you meet your basic needs
  • HIV and sexual health services
